OACCPP Recognition Award 
For Outstanding Service/Contribution
In the Field of Mental Health

The Ontario Association of Consultants, Counsellors, Psychometrists and Psychotherapists (OACCPP) is seeking nominations of someone you feel provided outstanding service and/or made significant contributions to the field of mental health.

 

Nomination Guidelines and Form

Eligibility 

Eligible nominees may include members of OACCPP or others professional colleagues who are not affiliated with OACCPP. The nominee's contributions based on their leadership, advocacy and excellence in their practices must be highlighted to qualify for this award.


Nomination Process

Provide a description (between 600 – 800 words) that summarizes and highlights the achievements and activities of the nominee in relation to this award. Please include references to the following criteria, which will be used to evaluate the candidate's:

Leadership: How has the nominee demonstrated outstanding leadership in the community, or in the workplace, that promotes mental health wellness? Describe verifiable examples that have also had an impact on a local and provincial level.

Advocacy: How has the nominee’s advocacy led to reducing barriers to those with mental health issues? List examples of practices and activities that have promoted awareness of mental health wellness. Cite specific examples.

Excellence: How has the nominee demonstrated vision, creativity and initiatives in mental health promotion or practices? List specific policies/practices in the community or workplace settings. Describe how the nominee has made a significant difference to the mental health community because of their activities.

You may also include other supporting documentation such as letters of reference.

Presentation: The award is presented during the Annual General Meeting (AGM). The OACCPP Executive reserves the privilege to not present the annual award.
